The Australian Government’s Support at Home program replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025. It provides coordinated services, products, equipment and home modifications to help eligible older people remain independent at home.
Following an aged care assessment, you receive a support plan listing your approved services and, where applicable, a funding classification. Our team works with you to plan and deliver care that reflects your needs, goals and preferences.

Support to maintain relationships, attend appointments, take part in community or cultural activities, and stay connected with the people and places that matter to you.
Approved transport services can help you attend medical appointments, shop, access community activities and stay independent.
The AT-HM scheme provides separate needs-based funding for approved equipment, assistive products and home modifications that improve safety and independence.
Approved respite services provide temporary support for you and give family or informal carers time to rest and recharge.
